The best method has always been to ask people you trust such as your family, friends, doctors and colleagues but you are still faced withthe lack of confidentiality and scope. I mean, if you have a personal problem, you don't go telling all your friends yet how do you ask for their recommendations. It isn't a problem with minor issues like a dentist or a chiropractor but certain types of diseases make it difficult to ask for recommendations from your friends. Another hurdle is scope. I can only ask my friends and ask them to ask their friends. This is clumsy, time consuming and lacks confidentiality.

It seems that every doctor that I see has an award of Best Doctor on their wall. There is no shortage of listings, reviews, and rankings on best doctors, both online and in print. The walls in doctors offices are not large enough to display all the wards yet the lack of credibility persists. You don't know who is doing the reviews, or who is ranking the doctors.

There are a dozen leading websites that rate and compare doctors. The first problem that patients looking for doctors are faced with is fraud. Most of these sites don't even require that users log in. Some allow anyone to rate any doctor with a single click. They can also rate as often as they like. This only generates worthless ratings. Sites that take the next step of making users log in only require that you provide an email address which does little to reduce fraud. The second issue with these leading doctors rating websites is who is rating these doctors? Do I trust my health and potentially my children's lives to some random star rating from anonymous internet users? No.

Magazines promising to help you find the best doctors always generate sales but fail to deliver on their promise. Print media is so limited since you cannot compare doctors, check credentials or perform further research.

Pill Camera Records Video of the Inside of Your Intestinal Tract

Wireless capsule endoscopy (WCE) is a relatively new and under utilized technology. It is under utilized due to limited insurance coverage for the test. Many people have heard about the so-called "camera pill" but do not know when it can or should be done, or how it works. Many mistakenly believe that it might be a substitute for colonoscopy, a flexible lighted tube with a camera on the tip used to examine the large intestine. Colonoscopy is recommended to screen for colon cancer and the precursor of colon cancer, colon polyps, as well as a diagnostic exam for evaluation of intestinal bleeding, diarrhea or lower abdominal pain. WCE is not yet technically feasible for examination of the lower or large intestine known as the colon. Given Imaging was the first to develop this technology.

The pill camera is activated by popping it out of a magnetized blister pack. It is swallowed with a glass of water. The patient is typically asked not to drink anything except water for 2 hours so that the camera pill has time to exit the stomach before being exposed to food or drink that could impair the video recording. Usually, a light lunch is allowed after four hours. At the end of the day the patient returns to the office to turn in the recorder so that the video can be downloaded to a computer for viewing by the doctor.

WCE is being tested for examination of the colon but the larger diameter of the colon, presence of stool, and longer transit time are all technical limitations that have delayed the development. Several researchers, including a team at MIT, are working on a remote controlled pill camera with the ability to sample tissues and deliver treatments. One investigator is trying to create a pill camera that reproduces the movements of a beetle, effectively crawling through the intestinal tract, stopping and backing up under robotic remote control. The future looks promising and resembles The Fantastic Voyage through the human body. Until then, WCE is very helpful and appropriate for the evaluation of certain gastrointestinal conditions and the signs and symptoms as described above.

WCE is however the only way to directly visualize the much longer and smaller diameter of the small intestine.Typical reasons for wanting to see the small intestine include unexplained intestinal bleeding, anemia (low blood count), suspected Crohn's disease, tumors, or known polyp syndromes. Of the approximately 24 feet of small intestine a person has, only the upper 1 1/2 to 3 feet can typically be reached and seen by a scope from the mouth and only the last few inches to a foot can be seen at the time of a colonoscopy exam through the rectum. It has long been thought and taught that very few serious diseases occurred in the small intestine (cancers or inflammation like Crohn's or Celiac disease) that could not be diagnosed by upper or lower endoscopy or small intestine barium x-rays. Since WCE has begun to be used more frequently, more disease has been found than was previously thought to occur.

Crohn's disease is a chronic inflammation of the intestinal tract that does not yet have a cure and is of unknown cause. It invariably involves the small intestine and often only involves the small intestine. Sometimes, the diagnosis cannot be made with conventional colonoscopy and/or intestinal x-rays. The extent of small intestine involvement is also commonly not accurately determined by x-rays. WCE is emerging as the diagnostic test of choice for Crohn's disease, especially when combined with specific blood tests and a colonoscopy.

Once activated, the small intestine pill camera battery lasts eight hours. Since the transit time from mouth to the end of the small intestine in most people is around 2-6 hours, this is more than adequate for the entire small intestine to be video recorded. However, some people have delayed emptying of their stomach, slow intestinal transit or areas of blockage or constriction that prevent the pill camera from reaching the end of the small intestine before the battery dies. When that happens no further recordings are transmitted. If the camera pill gets stuck, surgery may be required to remove it from the body. However, if this occurs, it is usually because a condition exists in the small intestine for which surgery is the treatment of choice.

The capsule endoscopy procedure requires little preparation. Most doctors request the patient fast overnight before coming into the clinic early in the morning. The patient has some electrical recording wires attached to the skin of the abdomen that record the location of the capsule as it passes through the digestive tract. This helps the doctor reading the video recording with interpreting where the pill camera is and if it is being delayed in an area. A video recording device is worn on a belt and wirelessly receives the video recording from the pill camera as it travels through the intestine.

Recently, a pill camera that has the ability to record video images from both ends and more rapidly, has been approved by the FDA and marketed as an alternative to standard upper endoscopy for examination of the esophagus. It is being marketed as a very quick, painless, and accurate method of screening for acid reflux, pre-cancer and cancer of the esophagus as well as varices of the esophagus. Varices are varicose type veins occurring in the esophagus of people with cirrhosis of the liver that if not treated with medication and/or rubber band ligation can bleed spontaneously and be life threatening. Insurance coverage for such examination is limited. Others limitations include the ability to only visualize the esophagus, leaving the stomach and upper small intestine unexamined in detail, and the inability to perform biopsies of abnormal appearing tissue. However, the ease and rapidity of the test appeals to people and will likely drive the popularity forward as long as insurance companies agree to cover the test.

Eosinophils and mast cells release chemicals that irritate the bowel, increase permeability (cause leaky gut), increase contractions of the gut, increase intestinal secretions and heighten pain. Both cells are related to allergies including food allergies. It is therefore not difficult to conceive of a link to adverse food reactions in the development of intestinal irritation.

Thousands of people are undergoing endoscopic exams daily without having tissue samples obtained. Sadly, though their exams may visually appear normal, under the microscope there are often microscopic findings that explain the symptoms that will respond to directed therapy. The gut is lined with superficial cells that contain a few immune cells that release chemical mediators that attract other cells to the area and fight off foreign invaders.

In the stomach and small intestine more than 10 eosinophils per high power field defines eosinophilic gastroenteritis. This newly recognized and described entity is previously unrecognized cause of diarrhea in some patients diagnosed with irritable bowel syndrome who may have been told they have a normal colon exam though no biopsies were done. Similarly, more than 20 lymphocytes per 100 epithelial cells in the colon are found in lymphocytic colitis, another form of microscopic inflammation of the intestine resulting in diarrhea that may be inappropriately diagnosed as IBS.

In the colon, the presence of eosinophils is considered one of the earliest findings of chronic inflammatory bowel disease. In the right colon more than 20 eosinophils per high power field and in the left colon greater than 20 per high power field is considered abnormal and suggests eosinophilic colitis, chronic inflammatory bowel disease or a parasitic infection.


Lymphocytes, eosinophils and mast cells are the immune cells that are normally present in small numbers in the surface cells of the gastrointestinal tract. A few lymphocytes are present in the tips of the surface cells that are a type of epithelial cell. These lymphocytes act as the body's scouts. They survey the barrier of the gut to the inside of the body, looking for signs of potential invading infectious agents. Once an attack is perceived, they signal reinforcements to join them on the front lines.

The important point to be aware of if you have gastrointestinal symptoms and are undergoing or have undergone an endoscopic examination is that a normal appearing intestinal lining does not exclude the presence of damage or irritation sufficient to cause symptoms of pain, bloating, gas, and diarrhea nor exclude impaired digestion and absorption. Blood tests exist that can help screen for celiac disease, Crohn's disease and ulcerative colitis but biopsies of intestinal lining is usually required for definitive diagnosis.

Eosinophils and mast cells are types of immune cells involved in allergy reactions in the body. They are less commonly present in the gastrointestinal lining except when there are parasites, food allergies, or chronic inflammatory bowel diseases such as Crohn's disease or ulcerative colitis. Eosinophilic gastrointestinal disorders are less common and a newly recognized condition, mastocytic enterocolitis, is diagnosed when excess mast cells are present in the small bowel and colon. However, mast cells may be difficult to see on biopsies without a special stain for tryptase, an enzyme present in mast cells that are immunologically activated.

Only through obtaining tissue samples that are examined under the microscope can abnormal types and number of inflammatory cells be identified. It is through biopsies of normal appearing intestinal lining that the correct diagnosis of various microscopic forms of gastrointestinal inflammatory diseases is confirmed. So, if you are preparing to undergo an endoscopic exam, I encourage you to insist that your doctor perform biopsies even they believe your exam looks normal. Based on the information I have reviewed above, a normal exam should be tip off that one of these microscopic conditions might be to blame for your symptoms.

SPECT imaging technology appears to be one exciting tool available to us if we can get the research funded. We will continue to explore the gut-brain connection further.

SPECT brain imaging of the majority of the few celiac disease patients studied reveals abnormalities that are usually most severe in the frontal areas of the brain. Improvement of these abnormalities are seen on a gluten-free diet. The frontal area of the brain is important in brain function that controls attention, impulse control, organization, and problem solving. Problems in this area of the brain result in short attention span, disorganization, procrastination, short-term memory problems, anxiety and depression.

The most dramatic report I have found comes from a 1997 report of a newly diagnosed celiac diease patient with established schizophrenia whose symptoms and abnormal SPECT scan reversed on a gluten-free diet. He presented with an established diagnosis of schizophrenia, diarrhea and weight loss. Endomysial antibody was positive and villous atrophy was present on intestinal biopsy. SPECT scan was performed before and after gluten free diet. Before GFD, the scan confirmed abnormal decreased blood flow to the frontal lobe of the brain. With a GFD schizophrenia symptoms resolved, the intestinal lesion resolved and the SPECT scan became normal. More recently in 2004, Usai et al. reported 34 celiac patients in whom 70% had abnormal SPECT scans. Again the abnormalities were most pronounced in the frontal areas of the brain and were less severe on a gluten free diet.
